My Visit

There are several ways to request an appointment: 

  • Patients can call the office to schedule an appointment. 
  • If you are an existing patient, log on to your Patient Portal account and select “Appointments.” 
  • Call your provider’s office to speak with a member of our team. You can find the office phone number by searching for your provider’s name. 

If you need to cancel or reschedule your appointment, we ask you to call your provider’s office to inform them in advance. Your provider may have additional guidelines regarding cancellation.

An email notification about your visit will be sent 24 to 48 hours before your appointment. You may need to fill out some forms prior to your appointment. If requested, please update your health history, demographics, your current insurance information, and place a credit card on file. 

On the day of your appointment, plan to arrive 30 minutes before your appointment to complete any outstanding check-in paperwork. You can also check in online on our patient-friendly portal, which may save you some time.  

If you have insurance, please bring a copy of your insurance card to every visit. We will submit the claim to the insurance company on your behalf. Your co-pay and any deductible your plan requires will be paid on the day of your visit.

After we receive payment from your insurance carrier, any additional amount determined by your insurance company that is your responsibility will be billed to you. 

Medical forms can be found on the patient portal or your provider’s office page.  

Our patient portal makes it fast and easy for you to be notified of your test results. Once enrolled in our patient portal, you may retrieve your lab results directly from the portal. 

The Patient Portal allows you to access your health records, pay your bill, fill out medical forms before coming in, view test results, double-check appointment dates/times, and more. Enroll now.  

Billing & Payments

Florida Woman Care makes paying for medical care easy by offering secure online bill payment through our patient portal. It’s very easy to set up a portal account. 

The cost for your visit will be determined based on the specifics of the care and services provided. Your insurance carrier may pay for all, or part of the care provided. However, you are primarily responsible for payment for all the services you receive. 

If you have any questions about your medical bills, please call your provider's office, and they will assist you. 

Insurance

Florida Woman Care prides itself in providing care to as many members of our community as possible. That’s why we do our best to accept most insurance plans. Each of our locations provides a list of insurances they accept. You can also contact your provider’s office to confirm that your provider takes your insurance.
